KMYZ “Last. Contest. Ever.” Official Rules
1. Grand Prize: 1 - $250 Visa Gift Card courtesy of Warner Brothers.
2. Contest open to U.S. Citizens that reside on the state of Oklahoma and are 21 years of age of older. Qualifiers and Winner must have a valid government issued Oklahoma state photo ID to prove residency. Listeners that have won a prize in the last 30 days are not eligible to win/qualify.
3. Listen to KMYZ-FM weekdays (Monday – Friday) from December 14th – 20th, between 9am and 5pm, when the song “It’sthe End of the World as We Know It (And I Feel Fine)” by R.E.M be the 10th callerat 918-460-1045, win a couples package to the KMYZ New Year’s Eve Party at the Hyatt Regency Tulsa on 12/31/12, includes hotel room, 2 party tickets and free parking, valued at $299 and qualify for the grand prize. Up to 6 people will qualify for this contest. One qualifier per household!
4. On Friday, December 21st, 2012 an employee of Shamrock Communications will randomly draw a grand prize winner from all eligible qualifiers. Grand Prize winner will be contacted by phone or mail.
5. Winner can claim their prize at the KMYZ studios. Winner must present a valid government issued when claiming their prize Monday – Friday between 8:30am – 5:30pm at the KMYZ studios, 5810 E. Skelly Dr. Ste. 801 Tulsa, OK 74135 during normal business hours. Winners must claim prize(s)/vouchers by Friday, December 28th by 5:30pm, No Later!
6. Prizes are non-transferable and cannot be substituted for another prize at the request of the winner.
7. Winner agrees to sign a release for use of their name and likeness for promotional purposes and they indemnify and hold harmless Shamrock Communications and their participating sponsors. All winners/qualifiers must have a valid credit card to use for hotel incidentals. Winners must pick up prize voucher at station and use code on voucher to make reservation.
8. Winner is responsible for all applicable local, state and federal taxes
9. Employees and families of Shamrock Communications, Warner Brothers Records, broadcasting media outlets in the Tulsa, OK area and other participating sponsors are ineligible.
10. Shamrock Communications reserves the sole right to substitute the prize(s) for one of equal or greater value or to modify, cancel or suspend this promotion, should fraud or other causes beyond the control of Shamrock Communications corrupt the administration, security or proper conduct of the promotion. Shamrock Communications is not responsible for any lost or stolen registration forms or boxes.
