KMYZ / MUSE Flyaway - OFFICIAL RULES
1. Grand Prize: Trip for 2 to Las Vegas ito see MUSE in concert on March 17th, 2013, includes 2 concert tickets, 2 nights hotel stay and roundtrip airfare for 2 from Tulsa, OK to Las Vegas, NV courtesy of QPrime, total approx. value $1,000.  (Leave 3/16/13, Return 3/18/12)

2. Contest open to U.S. Citizens that reside on the state of Oklahoma and are 21 years of age of older. Winner must provide a valid Oklahoma government issued photo ID as proof of Oklahoma residency.

3. Listen to KMYZ weekday from 10/8/12 - 10/12/12 (Mon.-Fri.) around 8am the MUSE song of the day will be announced. When the song of the day plays between 9a, - 5pm, that day be caller 10 at 918-460-1045 to win a pair of tickets to MUSE at BOK Center on March 10th, 2013 and be qualified for the Grand Prize.

4. One winner/qualifier per person and per household. Up to 15 will qualify for the grand prize. If you have won a prize from KMYZ in the past 30 days, you are ineligible.

5. On or around Monday, October 15th, 2012 a member of Shamrock Communications will randomly draw a grand prize winner from all eligible qualifiers. Grand Prize winner will be contacted by phone, e-mail or mail.

6. Winner must claim prize by within 30 days of being notified (or when trip materials are ready to be claimed at station) If the winner doesn’t claim the prize/voucher by the deadline a random drawing from all remaining eligible qualifiers.

7. Winner can claim their prize at the KMYZ studios. Winner must present a valid government issued when claiming their prize Monday – Friday between 8:30am – 5:30pm at the KMYZ studios, 5810 E. Skelly Dr. Ste. 801 Tulsa, OK 74135 during normal business hours.

8. Prizes are non-transferable and cannot be substituted for another prize at the request of the winner.

9. Winner and guest agree to sign a release for use of their name and likeness for promotional purposes and they indemnify and hold harmless Shamrock Communications and their participating sponsors. All participants must be 21 or older! Winner is responsible for all ground transportation to and from the airport, hotel and concert. Winner must provide a valid credit card for hotel incidentals.
