KMYZ, “iPad A Day in May”- Official Rules
1. Prizes:  One (1) 40GB Sony Playstation 3 with 1 controller, valued at $150.

2. Contest open to U.S. Citizens that reside on the state of Oklahoma and are 16 years of age of older.  Must have a government issued Oklahoma state photo ID as proof residency.

3.  How to enter
 A) Enter to win:   Enter to win during Tulsa Community College Student Activities Office business hours at participating sponsor locations, TCC Metro Campus (909 S. Tulsa, OK 74107) by Close of Business on Friday, January 18^th, 2013.  Register to win by completing an official entry form by printing your Name, Address, Phone Number(s), date of birth, e-mail address, signature and date and depositing it in the official registration box.  One entry per person.  Must register in person.  No mechanically reproduced entries will be accepted.

B) Mail in: On a 3”x5” piece to paper, print your first and last name, address (No PO Boxes), city, state, zip, phone number(s) (Home and Cell preferred) and date of birth (mm/dd/yyyy) and place in a stamped envelope and mail it to KMYZ “TCC PS3 Giveaway” 5810 E. Skelly Dr. Ste 801. Tulsa, OK 74135.  One entry per person, per envelope and no mechanically reproduced entries will be accepted.  You may only enter once by mail-in.  Shamrock Communications (KMYZ) is not responsible for any lost, stolen or misdirected mail.  All mail-in entries must be received by 5:30pm on Friday, January 18^th, 2013 All mail-in entries will be added to the on site entry forms.

4. A member of Shamrock Communications will randomly draw a winner on or around Tuesday, January 22^nd, 2013.

5. Winner must claim prize within 30 days of winning. If winner(s) should fail to claim prize within 30 days, they will forfeit the prize.

 6. Winner must claim their prize at the KMYZ studios. Winner must present a valid government issued when claiming their prize Monday – Friday between 8:30am – 5:30pm at the KMYZ studios, 5810 E. Skelly Dr. Ste. 801 Tulsa, OK 74135 during normal business hours. 
 
7. Prizes are non-transferable and cannot be substituted for another prize, or cash at the request of the winner.

 8. Winner agrees to sign a release for possible use of their name and likeness for promotional purposes and they indemnify and hold harmless Shamrock Communications and their participating sponsors and their respective affiliates, subsidiaries, joint venture partners, advertising and promotion agencies and the directors, officers, employees and assigns of each, harmless against any and all claims, damages, costs and liability arising out of acceptance/use of prize.  Winner assumes all liability for any injury or damage caused, or claimed to be caused, by participation in this promotion or use or redemption of any prize.  Sponsor and their Promotional Partners are not responsible for any typographical or other errors in the printing of the offer, the announcement of any prize, in the administration of the promotion or lost or stolen entries forms or registration box(es). 
 
9. Winner is responsible for all applicable taxes 

10. Employees and family members of Shamrock Communications, participating sponsors, and broadcasting media outlets in the Tulsa, OK area are ineligible.
 
11. Station reserves the sole right to substitute the prize(s) for one of equal or greater value or to modify, cancel or suspend this promotion, should fraud or other causes beyond the control of Shamrock Communications corrupt the administration, security or proper conduct of the promotion.  Prize comes as is.
 
12. Shamrock Communications is the sole judge of all matters pertaining to this promotion and their decision is final. All entries become property of Shamrock Communications.
